Where requested, we liaise with those closest to you, in order for your care plan to be tailored to your individual needs.
All I Care’s client base are deemed to have equal access to the above services as appropriate, without discrimination.
I Care will endeavor to work closely with external agencies e.g. Doctors, District Nurses etc by sharing information within the confines of the Data Protection Act 1998. I Care will take an active part in discussions with social services and clients’ next of kin during care review meetings, to facilitate the appropriate provision of care. Where there are concerns over an individual’s welfare, I Care will liaise with the appropriate authorities and/ or clients’ families.
When I Care receive a referral, an initial Care Assessment will be carried out with the individuals, their friends/ families and where appropriate an advocate or social worker. The care plan is created with maximum input from the client, as they choose who cares for them and how and when they want their care to be provided.
As in compliance with I Care’s Quality Assurance Policy, I Care endeavor to carry minimum three monthly periodic reviews to individuals’ care plans and make changes where necessary, with full consultation of the client.
Further to the above, if there is a positive or negative change to the clients’ circumstances outside of the three months, then they will be wholly included in a review of the care plan.
In line with I Care’s Responsive Services Policy, clients will have continuous access to communication with management and as such, any concerns are immediately addressed, e.g. the preference of Carers.
